The occurrence and incidence of chronic diseases continues its rise significantly, with aging demographics and the prevalence of harmful way of live were two of the most important risk factors contributing the aggravation of long-term illnesses affecting a large number of people, thus resulting to major human and economic costs to health systems in the world. The deteriorating situation in chronic diseases will create more multifaceted care administrations, undesirable drug procedures, repetitive testing, needless hospitalizations, incompatible medical recommendations, and malfunctioning conditions.

The ICT infrastructures have the ability to advance remotely accessible care services in normal basis, thus greatly impact health care procedures, improved results and can be readily adopted on regular basis.
